Protecting your health information

We are committed to protecting the health information of our patients. This notice explains how we may use and disclose protected health information (PHI), your privacy rights in your PHI, and our obligations concerning its use and disclosure.

How we may use and disclose your PHI

  • Treatment: We may use your PHI to provide, coordinate, or manage health care and related services by one or more health care providers. For example, we may request laboratory tests and use the results to reach a diagnosis, write a prescription, or share information with a pharmacy.
  • Payment: We may disclose your PHI to obtain reimbursement for services, confirm coverage, conduct billing or collection activities, and perform utilization review. For example, we may contact your health insurer to confirm eligibility or determine whether it will cover treatment.
  • Health care operations: We may use your PHI to operate our practice, including quality assessment and improvement activities, auditing, cost-management analysis, and customer service.
  • Appointment reminders: We may use and disclose your PHI to contact you and remind you of an appointment.
  • Electronic transmission: Our practice may include the office name, address, and patient-identifiable information in electronic insurance claims and statements.
  • Family and friends: We may release your PHI to a friend or family member involved in your care or assisting with your care.

Certain special circumstances

We may use or disclose PHI as permitted or required by law for public health activities; reports of suspected abuse, neglect, or domestic violence; health oversight; judicial and administrative proceedings; law enforcement; preventing a serious threat to health or safety; government functions such as military service or national security; emergencies and disaster relief; workers’ compensation or similar programs; and other instances required by law.

Patient concerns and complaint resolution

If you have a concern or believe we may have violated your privacy rights, please bring it to our attention immediately by contacting our Privacy Officer. You may identify yourself or remain anonymous.

We take privacy concerns and complaints seriously and will investigate each one promptly. If we made a mistake, we will do what we can to correct it and take steps to prevent it from recurring. If we did not make a mistake, we will provide an explanation unless the concern was submitted anonymously. We will make every effort to complete our investigation within 30 days.

We will not retaliate against you for expressing a concern or filing a complaint relating to your privacy rights.
