User Data Deletion Instructions
Use these instructions to ask Dr Tam Medical Spa Clinic to delete personal information you submitted through a public website contact or consultation form. The website currently delivers those requests to the clinic’s designated email inbox; it does not create a patient account or store the submission in the Content Studio database.
How to submit a request
- Email info@drtam-medspa.com with the subject “User Data Deletion Request.”
- Include the full name and email address or phone number used in the original request, the approximate submission date, and the channel where you interacted with the clinic. Include the website Lead ID if you have it.
- Describe the information you want reviewed for deletion. Do not include medical details or send identity documents in the initial email.
What happens next
The clinic may ask for limited additional information to verify that the request relates to you. Please do not send sensitive documents unless the clinic explains why they are needed and provides an appropriate way to send them.
After verification, the clinic will review information under its control and respond as required by applicable law. The response will explain whether the information was deleted or de-identified, or whether particular information must be retained.
Scope of these instructions
These instructions cover information submitted through this public website. If your request also concerns Facebook, Instagram, X, another platform, or another clinic system, identify that platform or system and the profile or contact details you used.
The clinic can act only on information it controls. A platform may require a separate request for information held in your platform account. Requests involving patient or medical records may also follow a different legally required process, which the clinic will explain when applicable.
